Schedule a Service

 You hereby authorize ALED Technologies, Inc to charge the credit card you have supplied for payment of fees, costs and expenses which are incurred by ALED Technologies, Inc services, which will include material fees actually spent and a minimum of 2 hours for labor regardless of the time actually spent by ALED Technologies, Inc service personnel.
Two hours of labor will equal a charge of $300 plus tax.
In the event ALED Technologies, Inc estimates the material fees to exceed $100, ALED Technologies, Inc will give you advanced written notice of the estimate and seek your approval before performing the services. You certify that you are authorized to sign this form.
